AIM created the Skilled Nursing division in 2014, initially as a response to augment case management’s ability to facilitate discharge planning in Wilkes-Barre. It was also created as a way to decrease readmission risk by continuing management of higher risk patients on the outside of the hospital post discharge. The demand for nursing home services continued to grow as more private physicians became less available to take care of nursing home
patients. By placing full-time APCs in multiple regional skilled facilities, AIM has been able to create a cost-effective model of high risk patient management post discharge. AIM now manages patients at ten nursing homes and two personal care homes in three counties of NE PA.

With changes in health care reimbursement such as MIPS & MACRA shifting an increasing amount of reimbursement to quality based payments, skilled nursing care has become a key component of cost containment, as skilled nursing facilities and other facilities such as LTACs were identified as large sources of waste in the health care system beyond the hospital’s walls. Our hospitals have been very supportive of our efforts to manage these patients in the nursing home setting to reduce costs, as well as reduce readmissions.
